Provider Liason / 282 Position Description:DESCRIPTION:The Provider Liaison serves as the liaison between APS’ disease management programs and providers and the community at large• Visits local providers/physicians to introduce disease management and supports/services provided by the program• Distribute tools and evidence-based clinical materials to providers/physicians in order to assist with quality improvement efforts• Respond to physician/provider inquires related to the program; and ensure physicians are integral components of each member’s care management plan• Provide provider education in consultation with Medical Director and outside consultants• Participate in community events that promote health and wellness and introduce APS’ disease management program• Perform work on special projects• Must be able to travel weekly. • Other duties as assignedEDUCATION REQUIREMENTS: Bachelors degree in related healthcare fieldEXPERIENCE: • Excellent verbal and written communication skills• Two years experience in community health or mental health care setting working with Medicaid members and providers• Experience in working with physicians, healthcare professionals, families, consumers and other social service systems of care• Computer knowledge required, including proficiency in MS Office Suite• Ability to manage multiple projects• Excellent organizational and time management skills
