Qualified Professional (Case Manager)

QUALIFICATIONS:

Masters Degree in a human Service Curriculum and one year  post graduate experience working with the Mental Health population OR Bachelor’s Degree in a Human service curriculum and two years  post graduate experience working with the Mental Health diagnosed population OR Bachelor’s Degree in non Human service curriculum and four years post graduate experience working with the Mental Health diagnosed population per 10 NCAC 27.G.0104 guidelines.

 

 

 

JOB RESPONSIBILITES:

Professional level providers who meet the requirements specified for Qualified Professional status according to 10A NCAC 27G.0104 and who have the knowledge, skills and abilities required by the population and age to be served may deliver Community Support as follows: coordination and oversight of initial and ongoing assessment activities; initial development and ongoing revision of Person-Centered Plan; monitoring of implementation of Person-Centered Plan.  Qualified Professionals perform service coordination activated within the established Person-Centered Plan, referral linkage, skill building, supportive counseling, and input into the Person-Centered Plan modifications.  QP is also responsible for the review and approval of billable hours and employee payroll hours.

 

DOCUMENTATION REQUIREMENTS:

Minimum standard is a daily service note that includes the recipient’s name, Medicaid identification number, date of service, purpose of contract, describes the provider’s interventions, includes the time spent performing the interventions, effectiveness of the intervention, signature and credentials of staff providing the service.
