Practice Quality Coach
Physician Health Partners, a premier health care management provider, is seeking a quality and customer focused Practice Quality Coach who will be responsible for building leadership and systems at practices to support sustainability in process and clinical outcomes. Function as a change agent at provider practices. Demonstrate effective project management and facilitation of individuals and teams.
Requirements:
The right candidate will have functioned as a “change agent” and has experience with problem solving operational and system barriers preferably in a health care setting. Identify and build leadership and QI champions at practice and embed QI activities into larger health care systems. Lead, model and train leadership attributes.
Guide teams through improvement process and impart knowledge of tools and techniques to create sustainable systems. Demonstrate the ability to build internal relationships but effectively push team out of comfort zone when necessary. Lead Planned Care Model Sessions at Practices. Implement Disease Registry at Practices, re-design practice work flows around the use of the registry.
The successful candidate will have strong experience with instruction and design of clinical and non-clinical communication and education regarding process improvement or quality initiatives.
Additionally, PHP is looking for someone who has strong project management and great relationship building skills.
· Minimum of five years experience preferably in Quality and Process Improvement in the healthcare setting.
· Bachelors degree in related field, Masters in Public Health, Health Administration or Social Work preferred.
· Strong computer skills, including data entry, word processing, and spreadsheet manipulation. Experience with Access preferred.
· Knowledge of Planned Care Model (aka Chronic Care Model) and patient self-management principles
· Knowledge of provider office work flow processes, health care delivery systems and health care trends.
