REGISTERED NURSE ASSESSMENT COORDINATOR-RNAC
Registered Nurse Assessment Coordinator responsible for management of MDS schedule, coordination of departmental input of MDS data for their sections, responsible for input of all nursing information for MDS purposes and final sign off as RNAC. Responsible for submission/transmission of MDS data to the Myers and Stauffer PA database as per required timelines for submission. Responsible to coordinate care delivery between nursing and therapy departments to ensure case mix maximization. Primarily Medicare population in 30 bed short term rehab focused SNF. Acts as the DON in the absence of the DON. Must have at least 1 year experience as MDS Coordinator responsible for all functions related to MDS schedule, creation and submission. Position is 64 hours per two week pay period.
