REGISTERED NURSE - TEAM LEADER

Roze Room Hospice a Medicare certified/JCAHO hospice program has an opening for a Team Leader.  The Team Leader fills a management position with both clinical and management responsibility for the team; serves as liaison to and conduit between the Director of Patient Care Services, clinical staff on the respective team, patients and their families and the community.

- Serves as resource and support system to staff in problem solving and determine apporpriate interventions.
-Conducts audits of team documentation, charts, admissions and discharges.  Reviews hospice documentation including care provider's notes.
-Leads the hospice IDT conferences while participating as a member of the team.
-Coordinates the IDT hospice services provided to patients and families.
