With expansion well underway in Racine and Kenosha counties and with new expansion into Waukesha County we are looking for RN Care Managers, Care Managers, and a Rehabilitation Specialist to join our growing community of caring.

Opportunities
RN Care Managers- to provide nursing care management services for our managed care program participants in Racine, Kenosha, or Waukesha County. These are full-time opportunities on days with no weekends, evenings or holidays. An RN degree or diploma, current WI RN license and two years of recent experience with older adults or persons with disabilities in a hospital, homecare or nursing home setting are required. BSN and community nursing experience are preferred. Car necessary for routine home visits.
Care Managers – to provide care management services for our managed care program participants in Racine, Kenosha, or Waukesha County. These are full-time weekday positions that involve routine in-home visits. A bachelor’s degree in a human services related field required. One year of recent experience with older adults or persons with disabilities in a social service capacity is preferred. Car necessary. Family Care or COP Waiver experience a plus.
Rehabilitation Specialist – a full-time opportunity to provide guidance, leadership and education regarding rehab services and durable medical equipment for our Family Care Program case management staff in Racine and Kenosha. This position is also responsible for assisting in the development of policies, procedures and practice standards for the provision of rehab services, identification of vendors and service providers, as well as discharge planning. A bachelor’s degree in OT or PT, current professional licensure in Wisconsin, and three years of clinical experience with older adults and persons with physical or developmental disabilities in a hospital, homecare, or nursing home setting required. A master’s degree is preferred.
