RN Case Manager - Hospice

Nursing is a core position per JCAHO and Medicare conditions of participation and performs skilled nursing assessments, manages the care plan, and coordinates services which will maximize the comfort and health of patients and families throughout the patient illness continuum. Nursing is responsible for identifying Hospice patient/family needs and for providing appropriate care and services in collarobartion with the attending physician's orders and the interdisciplinary plan of care. Graduate of an NLN accredited school of Nursing; BSN required; certified in palliative nursing preferrred. Current Pennsylvania Nursing license required. Current valid Pennsylvania Driver's license prequired.
