RN - Core Measures*

Eden Medical Center is a 387-bed, not-for-profit medical center with approximately 2000 employees at our two campuses. The Eden Campus in scenic Castro Valley includes Eden Medical Center, Laurel Grove Acute Rehabilitation Hospital and Baywood Court Skilled Nursing Facility. The San Leandro campus includes San Leandro Hospital. Eden is home to the regional Level II Trauma Center for Southern Alameda County, the Sutter East Bay Neuroscience Center, adult psychiatry, a wide range of rehabilitation services, a complete Birthing Center, Community Cancer Center, advanced imaging services and complete surgical and acute-care services.Through our affiliation with Sutter Health, Eden Medical Center is part of a large network of not-for-profit hospitals and care centers in Northern California and is linked to nearly 30 acute care hospitals and 4,000 physicians throughout the region. Together, we’re working to improve the health of our communities. Under general administrative guidance, functions independently across the Medical Center to assure patient in assigned populations/diagnostic groups receive care that consistently follows evidence-based standards.  Reviews the patient record to assess the need for education and medical staff intervention.  Assures documentation is evident that the patient has received optimal care according to the evidence-based standards, intervening with nursing and/or medical staff when necessary.  Captures information both in the medical record and through databases to meet the organizations expectations of process and outcomes monitoring.Education: RN license required, Bachelors Degree preferred, MSN optionalExperience: Must be familiar with databases and computer use.  Minimum of 3-5 years acute care experience or like experience in an outpatient setting.  Core Measures experience preferred.Skills: Excellent communication skills.  Knowledge base regarding evidence-based practice and related documentation requirements.
