Promotes excellent end of life care; collaborating with physicians and interdisciplinary team members; providing physical and psychological support to patients, and families in their end of life journey.

* Identifies patient care requirements by establishing personal rapport with potential and actual patients and other persons in a position to understand care requirements.

* Establishes a compassionate environment by providing emotional, psychological, and spiritual support to patients, and families.

* Resolves patient problems and needs by utilizing interdisciplinary team strategies.

* Maintains continuity among team members by documenting and communicating actions, and continuing needs.
 
* Contributes to team effort by accomplishing related results as needed.

Skills/Qualifications: Hospice experience preferred, home health care, nursing facility, and hospital experience considered.
