Yoh Health Care has a permanent opening for an RN Inpatient Case Manager in San Francisco, California.Responsibilities: • Assess, plan, implement, coordinate, monitor and evaluate the options and services needed to meet the member’s health needs across the care continuum and provide quality cost-effective outcomes. • Utilize clinical knowledge, assessment, problem solving and decision-making skills to evaluate requests for authorization. • Regular communication and interface with Physician Advisors and Medical Directors for clinical decisions regarding medical necessity, problem resolutions, and all care/service denials. • Regular interface with Health Plan representative on reporting clinical review as required by Health plan delegation agreement. • Work collaboratively with other members of the Case Management, Referral Services, Claims Department, Health Plans and other providers to identify individuals who require case management services. Requirements: • Licensed RN, BSN • Certification in CCM preferred • Minimum three years of case management preferred • Minimum 3-5 years working in an acute clinical setting • Excellent computer skills • Excellent communication skills Discover all that’s possible with Yoh Health Care.
