JOB DESCRIPTION:

PATIENT CARE COORDINATOR

INPATIENT UNIT

Position Summary

?? Responsible for the management of the hospice patient/family identified needs and services. Supervises patient

care. Responsible for IDT meeting, Plan of Care and the implementation of Plan of Care.

?? Reports to: Clinical Director

?? Supports hospice philosophy and the Provider Relations, Staff Development and the Quality Improvement

Qualifications

1. Current Registered Nurse licensure/registration in the state of practice.

2. Minimum of two years experience as a registered nurse.

3. Hospice, long term care and/or management experience preferred.

4. Thorough knowledge and understanding of palliative care. Certification in Hospice and Palliative Care preferred.

5. Excellent assessment skills.

6. Demonstrate leadership skills.

7. Excellent organizational/time management skills and problem-solving skills.

8. Demonstrate commitment to hospice philosophy of care.

9. Excellent oral/written communication and interpersonal skills; ability to comprehend written sentences and

paragraphs in work related documents.

10. Demonstrate active listening skills.

11. Must have reliable transportation, valid driver’s license, and the minimum state required liability automobile

insurance or access to reliable transportation.

Requirements

General:

1. Participate in the development and implementation of policies, procedures, protocols and best practices and

criteria which assure nursing services are delivered according to the standards established by the State Board of

Nursing.

2. Participate in the development of personnel policies and position descriptions that delineate qualifications, duties

and responsibilities for all levels of nursing staff.

3. Ensure nursing services delivered by hospice employees and by individuals employed through contractual

agreements are provided in accordance with established policies of the hospice and acceptable standards.

4. Approve time cards.

5. Participate in referral/admission process. Assure patient/families receive adequate care and needs and

appropriate services identified. Arrange for hospice physician consult to address assessment and symptom

control and hospice appropriateness.

6. Act as nursing liaison with hospice staff, other agencies, organizations and individuals which have contractual

agreements to provide nursing service.

7. Manages FTE productivity, drug utilization and medical supply costs through data reports, roadmaps, technical

guidelines and other tools on a daily basis.

8. Secure documentation to justify all patient certifications and recertifications and assures timely signatures.

9. Conduct the interdisciplinary team meetings and maintain appropriate documentation for incorporation into the

patient record.

10. Lead IDT utilizing CSC approved agenda and Misys roadmaps. Encourage participation by all team members.

11. Manages FTE productivity, drug utilization and medical supply costs through data reports, roadmaps, technical

guidelines and other tools on a daily basis.

12. Coordinate the development, implementation and review of the interdisciplinary Plan of Care for each

patient/family, and ensure each patient/family Plan of Care is professionally managed.

13. Ensure the initial assessment of a patient requiring the services of a registered nurse, licensed nurse, home

health aide, or other nursing support staff is performed by a registered nurse.

14. Ensure the number of nurses required to provide the nursing services identified in each patient’s plan of care are

sufficient and available.

15. Review and adjust nursing work schedules to insure that nursing services identified in each patient’s plan of care

are provided accordingly.

16. Ensure the Registered Nurse for each interdisciplinary group coordinates the implementation of the plan of care

for each patient assigned to that interdisciplinary group.

17. Communicate as needed with all members of the team, the patient/family unit, the Medical Director, the

Administrator and others involved in the care of the patient to facilitate optimal care of patients/families.

18. Ensure hospice records include comprehensive documentation for each service provided to patients/family

according to established standard operating procedure.

19. Participate on committees and special projects and other duties as assigned.

20. Initiate preventive and rehabilitative nursing procedures, as appropriate.

21. Ensure changes in the patient’s medical and mental condition are communicated to the attending physician as

well as to other hospice staff and contract employees involved in the patient’s care.

22. Manage the development and implementation of a discharge plan when a patient is discharged prior to death or

transferred to another hospice.

23. Ensure all patients are provided services at an appropriate level of care per payer source. Secure billing

accuracy through participation in pre-bill audit process.

24. Ensure current contract in place prior to patient placement in a facility at a level of care requiring a contract per

regulatory requirements.

25. Serve as the IV Therapy Supervisor by:

a. Assisting the attending physician to develop a Plan of Care through evaluation of patient needs and

the capability of the caregiver to cope.

b. Assisting the hospice staff, patient and caregiver to understand, accept, and follow instructions in the

Plan of Care for IV Therapy Services.

c. Ensuring that the appropriate IV Therapy Services are delivered according to the Plan of Care.

Further, ensuring that the patient, caregiver, and hospice staff know how to dispose of IV waste,
