RN - REGISTERED NURSE - CASE MANAGEMENT

This full-time position is will conduct case management program activities in accordance with departmental, corporate, state, federal and URAC accreditation standards.  Uses a systematic approach to identify members who benefit from case management.  Implements the health plan Case Management processes to facilitate quality, cost effective medical and benefits management.  Provides appropriate resources and assistance to members with regards to managing health care across the continuum of care.  Participates in interdepartmental coordination and communication to ensure delivery of consistent and quality health care services.

The ideal candidate with be an RN/Registered Nurse with a current New York State license.  Bachelor's degree preferred, CCM desired.  Three (3) years of progressive clinical experience including community health, behavioral health and managed care experience preferred.  Excellent interpersonal communication skills.  Computer experience and use of measurement/criteria.  Ability to travel.
