RN on Call-Hospice

Lighthouse Hospice is dedicated to providing comprehensive and compassionate hospice care. We strive to add to the quality and dignity of the lives of our patients and their families. Our goal is to make life easier, treating patients and their families as we would our own. We are seeking a Fulltime RN on Call for our Far West, Austin Texas Location. Shift is 5 pm Friday through 8 am Monday. Reports Directly to:  Patient/Family Services Coordinator/Coordinator of Clinical Services

Basic Purpose:

This Registered Nurse is a professional caregiver who assumes responsibility and accountability for a group of patients for a designated time frame and provides care to these patients via the nursing process with the assistance of other health care team members in accordance with established policies and philosophy of Lighthouse Hospice.

Qualifications: Graduate of a nationally accredited school of nursing with current licensure in the State of Texas or a current work permit from the Texas State Board of Nurse Examiners. Hospice experience preferred. Skilled in principles of team nursing, family-centered nursing, communication skills, interpersonal skills and confidentiality of patient information. Good mental health, stable personality, and ability to be flexible, creative, and to work effectively and compassionately with others. Valid driver's license with proof of at least the State required limits on the bodily injury liability section of their personal auto policy.

Principal Duties and Responsibilities:

Assessment. Collects, analyzes, and synthesizes data via admission and daily assessments of patients focusing on physiologic, psychologic, and patient/family cognitive status and learning needs.

Planning. Formulates an individualized plan of care appropriately delegating/coordinating tasks and duties to health care team members utilizing sound knowledge base for decision making and establishing priorities of patient/family care.

Implementation. Implements care in a knowledgeable, skillful, and consistent manner by proper application of psychomotor and cognitive skills, complying with established Policies/Procedures/Protocols. 

REGISTERED NURSE 

Evaluation. Evaluates effectiveness of care given by all health care team members including self as reflected in the documentation of patient's/family's progress/responses, communication skills, and problem identification and resolution.

Interdisciplinary Approach. Participates as an integral part of the Hospice healthcare team, promoting optimal care for each patient/family. Functions as Case Manager for assigned patients and families.

Quality. Actively seeks to attain and maintain the highest quality of patient/family care. Routinely participates in the Quality Assurance Program.

Confidentiality. Maintains privacy and confidentiality of patient, family, and team issues.

Documentation. Documents accurately on all forms available using the nursing process, correctly describing the present status/condition of the patient/family, treatments, medications, physician orders, and patient/family responses. Documents in a concise, careful, and timely fashion according to established policies.

Compliance. Conducts business according to the highest ethical standards and in compliance with federal and state laws and regulations. Complies with and promotes Lighthouse Hospice's Compliance Program including the "Standards of Conduct."

Personal/Professional Growth. Demonstrates responsibility and accountability for own professional practice participating actively in staff development and attainment of goals/policies, Quality Assurance/Risk Management activities and utilizing effective working relationships with all health care team members, patients, significant others, and community. Keeps abreast of current professional standards of the nursing field and makes recommendations for changes in Policies and Procedures, equipment, and programs.

Environmental Demands: General hospital environment. Home environment. Lifting and transferring of patients. 24-hour coverage of patient care. Walking, standing and bending. Exposure to infectious organisms.  Sensory skills to differentiate clinical signs. Driving. OSHA Category 1 – “All procedures or other job related tasks that involve an inherent potential for mucous membrane or skin contact with blood, body fluids or tissues, or a potential for spill or splashes of blood or body fluids.�
