RN or LPN NURSE LIAISON

Full-Time

Indian River Center, located in West Melbourne, Florida, is currently seeking a high-energy, self-directed, outcome-oriented RN or LPN to facilitate patient flow from hospitals and community settings into our skilled nursing centers. 

 

Responsibilities include:

- Maintaining referral relationships with physicians and hospitals

- Conducting pre-admission clinical assessments

- Assisting with the admissions process

 

A Florida RN or LPN license is required; experience in LTC, case management, healthcare marketing or pre-placement assessment is preferred.  Excellent critical thinking skills, assertive and proactive approaches to referral development, and quick decision-making are keys to success in this fast-paced career opportunity.
