Full time day position for an RN Case Manager in Southeastern Michigan. Provide care coordination, assessment and support to hospice patients and their families/caregivers. Work with an interdisciplinary team. Provide compassionate care and comfort. Communicate effectively with customers, patients, and internal team.

After Hours positions available as well.

Registered Nurse License required. Experience with hospice or related field preferred.
