Registered Nurse - Clinic Manager

INTERCEDE HEALTH - POSITION DESCRIPTION

 

 

POSITION TITLE:  RN, Clinic Manager/Educator

REPORTS TO: Clinic Administrative Director/Nurse Practitioner

 

 

PURPOSE OF POSITION:

To work collaboratively with physicians and nurse practitioners within the team setting to facilitate quality care for patients according to established Transitional Care protocols. In addition, incumbent will provide professional nursing care to clinic patients under the supervision of nurse practitioner in accordance to guidelines meeting or exceeding outcomes. Incumbent may be required to manage and oversee non-physician clinical staff, facility and daily operations of the Transitional Care Clinic.

 

ESSENTIAL FUNCTIONS AND RESPONSIBILITIES:

 

1.  Coordinates, provides and supervises patient enrollment, scheduling and workflow.

2.  Performs general nursing care in collaboration and under supervision of a physician/nurse practitioner.

3.  Assures and performs on-site and telephonic disease management.

4.  Performs patient/family education of disease process/treatment/management.

5.  Performs on-call duties as assigned.

6.  Provides and supervises non-physician clinic personnel and operations.

7.  Maintains and manages facility to meet laws, regulatory and/or client requirements.

8.  Demonstrates a working knowledge, adherence to contractual guidelines, and communicates effectively to case managers, clients, and par providers.

9.  Participates and attends management meetings, staff meetings and/or training or educational sessions.

10. Utilizes data, data collection tools, provides reports and implements corrective actions for operational and clinical performance issues.

11. Adheres to, actively participates in and contributes to quality improvement activities.

12. Meets and/or exceeds outcome goals/benchmarks.

13. Participates in mentoring and training of new employees, temporary / contracted staff, current employees or peers.

14. Other duties as assigned.

 

EDUCATION, TRAINING AND EXPERIENCE (Includes knowledge, skills and abilities):

 

Required: Current valid registered nurse license in the state of Texas

Current BLS / CPR certification.

  Minimum of 3 years of applicable clinical experience

  Proficient computer skills 

 

Preferred:  Baccalaureate Degree

Supervisor or Charge Nurse Experience

Bilingual

Disease Management and/or telephonic triage experience.
