CATEGORY: Clinical Case Manager POSITION SUMMARY Coordinates the development, completion and transmission of the resident assessment in accordance with the regulatory requirements. Reviews and manages reimbursement in compliance with SunBridge policies and procedures. Responsible to case manage residents covered by either Medicare or Managed Care insurance programs. Ensures that each resident's needs are met and exceeded while maintaining the highest degree of dignity. Requirements
RN in Massachusetts. Demonstrated proficiency in MDS/RAI/PPS processes Long Term care experience a must. Current CPR Certification
