Registered Nurse-Hospital Liaison in Lawrenceville!

DescriptionThe Hospital Liaison is responsible for building relationships that ensure that all eligible patients have access to VistaCare by facilitating referrals and admissions through daily interaction with hospital staff and will maintain case management responsibility for all patients receiving care in the facility (related, nonrelated, and respite). Principal AccountabilitiesClinical Component 1. Evaluates patients to determine eligibility. 2. Available to immediately admit referred patients at assigned hospital(s). Responsible for hospital needs as needed. Performs clinical assessments and develops a Plan of Care for patients admitted while in the hospital. 3. Delivers care according to the plan of care and coordinates care with the facility staff to ensure care is delivered according to the plan of care. 4. Notifies members of the patient care team about admissions and transitions patients onto service. 5. Coordinates care and discharge planning with hospital staff to maximize facility efficiency. 6. Coordinates VC support services to meet patient needs while hospitalized under GIP or respite. 7. Carries out general inpatient and respite nurse visits and case management with hospital patients. 8. Completes the Hospital Tracking tool for each patient. 9. Ensures compliance to corporate identity and admissions access standards.Marketing Component 1. Maintains key referral sources at assigned hospital(s) to their fullest potential. 2. Promotes VistaCare to attending physicians, discharge coordinators, and other hospital staff on a daily basis. 3. Implements VistaCare’s sales strategy and process. 4. Responsible for conversion of referrals to admissions from hospital referrals. 5. Coordinates with facility billing representative  to ensure smooth and timely billing and reimbursement. 6. Collaborates with area, regional and national sales/marketing team on an ongoing basis in the development of hospital partnerships and relationships. 7. Gives professional presentations using PowerPoint and VistaCare standard segment presentations. 8. Conducts bill analysis with OM, ED, PCM and DPR assigned to the account in efforts to monitor our effect on cost containment measures. 9. Attends Liaison meetings with the hospital when appropriate.General: 1. Adheres to VistaCare Standards of Conduct. 2. Has a duty to report any potential misconduct or violation of VistaCare Standards of Conduct or federal or state laws and regulations. 3. Has a responsibility to participate in staff training and education programs. 4. Performs other duties as assigned. Qualifications1. Two years nursing experience; one year of prior hospice or hospital experience highly desirable. 2. Registered Nurse with current state license. 3. Excellent communication and customer service skills. 4. Positive, “can do” attitude. 5. Knowledgeable about the hospital industry. 6. Good computer skills, including Microsoft Office, Internet and e-mail. 7.
