Rehabilitation Specialist (Direct Care)
Position Title: Rehabilitation Specialist
Report to: Program Manager
Job Summary:
Part- and full-time Rehabilitation Specialists needed for all shifts in the Newton and Waltham area. The Rehabilitation Specialist provides direct care services to adults with Traumatic Brain Injury (TBI) living in SHIP and DMH residential programs in a manner that promotes client's safety, stability, health promotion, the development of independent and community living skills and an improved quality of life.
Essential Functions of Position:Develops and implements program activities which will foster the clients' growth, skill development and health promotion. Teaches daily- living skills such as safety rules, travel training, banking and budget management, cooking, shopping, personal care and hygiene and care of personal space and belongings. Coordinates medical, dental and other treatment appointments for the clients. Attends weekly house meeting, staff meeting, and supervision meetings as required by the supervisors. Maintains programmatic documentation such as Shift Log, Data Collection Log, Communication Log, etc., in accordance with agency standards. Implements ISPs and behavioral treatment plans. Provides crisis assessment, prevention and needed intervention. Accompanies by MBTA or transports clients to medical, dental and psychiatric appointments, leisure activities, etc. Assists clients with housekeeping tasks such as cleaning the bathrooms, kitchen, common areas, Bedrooms, yard work (including leaf and snow removal), closets, etc. Performs housekeeping tasks and routine cleaning as assigned on shift and by the supervisors. Provides skilled shift coverage as scheduled by the supervisors. Performs other duties and projects as assigned by the supervisors.
Qualifications:Previous work experience in human service setting or education in human services field preferred. Bilingual English/Khmer encouraged to apply. Strong desire to help others in need and willingness to learn new skills necessary for the job. Demonstrated ability to work effectively and collaboratively with staff, supervisors, other care providers, family and community members. Valid driver's license and good driving record.
