SOCIAL SERVICES COORDINATOR Mercy Health System Support Services (Corporate Office) and Mercy Elderly Housing has an immediate opening for a Social Services Coordinator. This position is part-time benefited and located at Mercy Court in Yeadon, PA. The Social Services Coordinator identifies frail and at-risk residents, provides general case management, comprehensive planning, and referral to appropriate services for the residents of Mercy Court. The Coordinator addresses the medical, social, and environmental factors which affect the resident and through intervention attempts to meet the individuals needs. Education, experience, and other requirements include: Bachelor's degree from an accredited school of Social Work or other related degree required with at least two (2) years of Social Work or Gerontology preferred; ability to communicate and work with staff and residents with diverse backgrounds and educational levels to prepare reports; ability to extend empathy and compassionate caring; also able to exhibit humor, maturity, and balance in handling difficult situations.
