NLACRC is now hiring additional case managers for our offices in VAN NUYS and ANTELOPE VALLEY [LANCASTER]. This is an excellent career opportunity for experienced SOCIAL WORKERS who want to work in a growing organization and who have the creative problem-solving skills to provide collaborative assistance to people who have developmental disabilities and to their families. Openings are available with populations consisting of 0-3 years [Early Start], 4-18 years [School Age], 21 years or older [Adult].



JOB DESCRIPTION:

Our social workers act as case managers, helping a group of clients and families find creative solutions to their service needs. Strengths in listening, problem solving, communicating information, and keeping track of work are a must, as is a positive outlook. We provide extensive training in the specifics of developmental disabilities. Some jobs require BILINGUAL SKILLS in SPANISH, or other languages. Additional compensation is offered for those jobs.



THE JOB: Initiate & respond to communications with clients & families. Provide clients & families with information & training on service issues, collaboratively develop service plans, manage the funding-approval process, follow-through on the plan's implementation & evaluation. Document all case activities, & maintain accurate case records. Teach self-advocacy skills to clients and families.



REQUIREMENTS: BA in a related subject plus two years related experience or MA in a related subject. There is regular local travel, so a valid CDL and use of vehicle or acceptable substitute required.



PAY, BENEFITS, APPLICATION: Salary: $3,176 - $4,093 per month, plus $150 bilingual payment, if required. Excellent benefit package, including contribution to the California Public Employees Retirement System.
