Senior Community Outreach Representative

HealthSpring takes an active role in building and maintaining the relationship between the health plan and the doctor by providing ongoing physician and office staff education.

 

Our Houston office is seeking a Senior Community Outreach Representative for our Community Outreach Department. In this key position you will report directly to the Manager of Community Outreach and will be responsible for coordinating and conducting community outreach activities along with corporate grassroots initiatives that place emphasis on member wellness and prevention while providing education that promotes a higher quality of life for Texas HealthSpring members and other Medicare recipients in the Texas HealthSpring service area.

 

To meet the challenges of this position you must possess knowledge of CMS Medicare Managed Care Manual rules and regulations.  Have the ability to work in a fast-paced, team oriented environment with minimal supervision; meet critical deadlines, be detailed oriented with excellent multi-tasking skills and efficient in MS Word, PowerPoint and Excel programs.  The successful candidate must have excellent interpersonal skills coupled with being an effective communicator. College degree or equivalent job experience in a Managed Care environment required.

 

The responsibilities of this position are to:

 Identify primary senior organizations and associated key management staff in assigned service area.Identified Senior organizations should include but not limited to: faith-based senior programs, social programs, assisted living facilities, civic activities, city councils, parks and recreation departments and senior centers. Develop and foster constructive working relationships with senior facility management staff.Identify senior service needs in assigned service area and develop community service action plans around those needs.Implement community service activities identified in the Community Outreach Plan.Maintain monthly senior activities calendar for the service area.Coordinate internal communications with Texas HealthSpring departments to educate staff on community service opportunities.Conduct Welcome/Education meetings for Texas HealthSpring members.Coordinate community service special events.Assist with coordination of Health Services Disease Management special events.Serve as liaison for Corporate identified grassroots initiatives.
