Job Description Senior Community Wellness Coordinator On behalf of the Health and Wellness Institute; Veritude is seeking candidates for a Senior Community Wellness Coordinator. This position will be located on-site at Wellmark Blue Cross Blue Shield location Sioux Falls,SD. Summary In this role you will Assist Wellmark members and clients in establishing and targeting their health management goals by connecting those audiences to resources available from Wellmark, its partners, and community/public outlets. In pursuit of delivering quality health management services to Wellmark members and clients via multiple channels, this position is responsible for working with appropriate Wellmark and HWI staff to assess client needs, communication channels, and health management opportunities. Accountable for the resource identification, program/service promotion, strategic and tactical planning, reporting and data interpretation for all assigned clients. Also responsible for the training, mentoring and quality assurance review of Community Wellness Coordinators to ensure optimal effectiveness and efficiency in both internal operations and external client relationship management. Responsibilities Participate in targeted resource identification, tactical execution, promotional outreach, client interaction, problem solving, cost analysis, data collection, reporting, member service, and deadline management to connect Wellmark members with health management resources across Wellmark’s Iowa and South Dakota service areas. This is accomplished through strategic planning and the development of strong relationships with and/or knowledge around: Wellmark’s internal health/lifestyle management units; Wellmark’s vendor partners; community health outlets; community organizations such as churches, sporting leagues, and chambers of commerce; and the client organization itself. Assist Wellmark members and small business clients as a strategic consultant, meet their personal/organizational objectives using sound analysis and established health promotion methodologies, and define concrete solutions supporting their health management strategy and/or budget. Facilitate Wellmark member access to WHD and community wellness resources by delivering community seminars, and serving as a point of contact for health management questions. Work with relevant Wellmark and partner stakeholders to connect members to the appropriate services and resources for their health management goals/needs. As appropriate, provide contact information to connect members to Wellmark disease management and case management programs, customer service, sales, or other relevant units/services. Collaborate with Wellmark Network Relations Managers to drive member engagement with network providers in support of the Medical Home relationship at the core of the WHD methodology. Provide impromptu service as needed by Wellmark stakeholders and meet with staff as appropriate to keep abreast of the latest developments to WHD and other health plan initiatives; work with other operational area managers to ensure all organizational objectives are met in a timely, efficient, cost-effective manner. Ensure that community events to facilitate member access to Wellmark/WHD/community health management resources are delivered efficiently and with high quality to individuals and client organizations. Ensure that all events/services/programs are delivered within established resource and time parameters. Interface with Wellmark small business clients to perform environmental audits and other data gathering activities as a precursor to developing strategic health and wellness organizational goals and recommending services from Wellmark or community outlets to achieve these goals. Train and mentor community wellness coordinators using HWI’s standardized policies and practices. Review and approve community wellness staff work related to presentations, proposals, recommendations and reports to ensure high quality service for all clients. Conduct quality assurance audits of community wellness coordinator performance and provide feedback to foster continuous improvement. Responsible for tracking and documenting all activities through the Microsoft Dynamics CRM to ensure appropriate facilitation of the Health Coaching and Community Wellness Coordination models. Enforcement of all staff to this adherence through CRM reporting and time tracking. Qualifications Bachelor’s Degree in Health Promotion/Education, Health Administration, Public Health, Health and Human Services, Business, Marketing, or related specialty and three-five years practical experience Three years of consulting, sales and/or account management experience Experience conducting needs assessments and preparing and presenting recommendations Experience in educating and counseling in a wide range of circumstances from large audiences to one-on-one health coaching sessions Ability to utilize strong persuasive skills to advise members/clients on best practices for implementation of applicable/appropriate health promotion programming Two to three years of consulting or account management experience is desired Master’s degree preferred Two- five years of certified training instruction is preferred Product development and project management experience preferredHWI values diversity and is an equal employment opportunity employer.
