As our newest Service Coordinator/Care Manager, you would visit patients in their homes or other community settings, acting as their advocate and helping them to stay healthy and independent in their current living situation. Bring your expertise to a thriving Fortune 50 Company and experience all the excellent benefits and advancement potential that this role has to offer; apply today Primary responsibilities of the Service Coordinator/Care Manager LPN include: Visiting patients in their homes, assisted living facilities and other community-based settings Delivering a holistic approach to coordinated care based on member needs Identifying early risk factors and conducting ongoing assessments Creating care plans and focusing on disease management Functioning as an advocate for members and decreasing unnecessary hospitalizations Collaborating with the member’s Physician to deliver and coordinate necessary services Building relationships with members and their families; assisting them with proper health choices Ensuring cases are documented in a timely manner Participating in team meetingsThe requirements for this role are as follows: An LPN with a current and unrestricted license in their state of residence Clinical experience A background in long-term care (could include: home health, hospice, public health, assisted living and community-based nursing) Computer proficiency Within the service area for DSL or CableAdditional assets include: Case management experience Bilingual Spanish or Native American and English speaking skills Medicare/Medicaid experience A background working in pediatrics special needs Experience working with patients with compromised respiratory function (using a ventilator) A background in behavioral health Experience with electronic chartingUnitedHealth Group offers a full range of comprehensive benefits to full-time and part-time employees, including medical, dental and vision, as well as a matching 401k and an employee stock purchase plan. Diversity creates a healthier atmosphere: equal opportunity employer M/F/D/V. UnitedHealth Group is a drug-free workplace.
