Education: BSN or BA in health care related field required. Masters degree preferred. Graduate of accredited school of nursing. Current California RN license required. CPR certificate required Licensure/Certification: CPR certificate required Position Summary: Manages various Continuing Care departments as assigned, which may include 24-hour and multiple location departments. Plans, organizes, directs, evaluates and coordinates inpatientnursing services and/or integrated outpatient- inpatient nursing services across the continuum ofcare within a Service Area or within a single medical center. Partners with the hospital on discharge planning and social issues. Coordinates with  TPMG and appropriate hospital staff to provide for the seamless transition of patients across the continuum of care. Works with KFHP/H and the Medical Group staff developing strategic plans, providing quality care, cost effective services which are aligned with federal, state, and local regulations.  Qualifications: Extensive experience in inpatient nursing (usually 5 years). Proven experiencedelivering results in both patient and management/leadership roles required. Knowledge of NCQAregulations, JCAHO standards, Nurse Practice ACT, NASW Code of Ethics and other local, state, andfederal regulations. Previous supervisory experience preferred (usually 4 years). Demonstratedstrong interpersonal communication, problem solving, conflict resolutions and team building skills.Background in discharge planning and concurrent utilization, strongly preferred. Skills Testing: N/A Duties: Manages the operations of 24 hr Continuing Care Department that may be at multiplelocations. Manages other departments as assigned. Supervises and develops the utilizationmanagement, discharge planning and social work team member and functions. Ensures staff provide thehighest quality of care and are in compliance with regulatory requirements.  Actively seeks opportunities to improve utilizations performance, social work practice andestablishes concurrent UM activities and social work strategies. Oversees discharge planning,Inpatient UM activities and social work. Together with the multi-disciplinary team, evaluatesprocess and makes implementations to improve resource management systems and patient care outcomes across the continuum of care. Directs the development and implementation of medical social work programs in the inpatient setting. Monitors utilization indicators; Identifies and escalatesprograms; and initiates and evaluates action plans for achieving the area's utilizationgoals/targets. Interprets utilization and regulatory requirements and standards and ensuresdepartment compliance. Focuses on prevention/intervention and identification of issues/problemsprior to their impact on operations and patient care.  Facilitates the interface of the following areas into UM/Social Work activities: Serve Area KFH/TPMGAdministrative Leadership, UM Chiefs, KFH and TPMG service leaders/directors, managers across thecontinuum of care, key individuals responsible for Medical Practice Guidelines, and leadersresponsible for UM activities associated with outside hospitals. Ensures compliance with all HRtracking and documentation requirements. Determines appropriate staff mix for department anddevelops/implements processes to screen, interview, hire, train and maintain competency of alldepartment staff. Ensures ongoing staff development. Develops, monitors budgets for appropriate useof human and material resources. Monitors financial performance and identifies and implementsstrategies to reduce cost and improve quality of care and service. Directs department qualityimprovement activities. Reports out on quality and utilization data as required.
