Social Services - Case Manager

Case Manager:

The Case Manager is the designated advocate for each client. It is their role to interface, network and communicate with referral agencies and community resources, as well as parents and other concerned family members. Case Manager ensures that quality clinical services are consistently and effectively delivered to each client assigned to their caseload. They are responsible for upholding regulations established by the Department of Social Services and the State of California.



RESPONSIBILITIES:

Collaborates with members of the multi-disciplinary treatment team for the delivery of a full spectrum of clinical services.

Assists clients in meeting individual treatment plans, goals and objectives for assigned cases. Provides individual counseling services.

Utilizes appropriate crisis intervention and de-escalation techniques in response to problems and/or behaviors which place the child or other residents in danger.

Communicates with parents and other approved family members. Encourages and facilitates family involvement in parent education, weekly visits, family recreation days, and other Phoenix House events.

Interfaces with referral agencies such as Juvenile Probation, Department of Children and Family Services, and Department of Mental Health.

QUALIFICATIONS:

Must possess a Bachelor’s or Master's degree in Psychology, Sociology, Social Work, or related

2 years of experience in social work activities

Bilingual (Spanish/English) preferred, but not required

Must meet federal, state and local Criminal Clearance and Child Abuse Indexing requirements.
