Social Worker-Adult Outreach Program

Overview:

The Intake Social Worker fill an administrative and telephone intake and referral role while providing services over the telephone to members of the fraternity over 60 years of age and their families. This person should be able to perform telephone assessments of the needs of the applicants, make referrals or reports as needed, assess for crisis, and guide the applicants through the application process. The Intake Social Worker will also be expected to perform office management duties, assist with reports, and must have computer skills.

Essential Functions:

1. Creates and maaintains reports on Excel and Word. Updates and maintains accurate information on the online access database, demonstrates thorough knowledge of the system

2. Creates check requests through our online system, organizes documentation, acts as on office adn telephone to Program and Care Managers in the field.

3. Performs phone assessment to evalute physical, functional, emotional, and enviornmental status of clients.

4. Provides information on care and community resources to callers

5. Coordinate paperwork and documentation for program applicants, working closely with applicant and family to obtain needed documentation

6. Keeps informed of eleder care trends, new programs and resources as source of information for clients and families.

7. Maintains office, filing, organization, orderign supplies

Minimum Qualifications and Experience:


Bachelor's degree in Social Work, Psychology, Gerentology or related fields from an accredited college/university Masters degree strongly preferred.





Must have excellent organizational and basic math skills





Knowledge of the aging process, health and social service system





Must be able to be clearly understood on the telephone by elderly callers.





Computer skills, Excel, Word





Must be able to exercise independent judgment, demonstrate good assessment skills and relate effectively to the elderly, professional associates, and clients’ families.
