Social Worker - Health

Coordinates comprehensive services for children, birth through twenty-one, who have developmental delays, disabilities, and/or chronic health conditions, and their families.  Prepares individualized service plans and compiles monthly statistics.  Provides information and referral to area resources.  Completes home visits, and attends meetings as required to fulfill job specifications.

 

Job Qualifications:  Bachelor degree in Social Work with certification by the Board of Social Work Examiners.  Must have a valid driver's license with own insured vehicle.  Bilingual preferred but not mandatory.
