Social Worker - Healthcare

Medica HealthCare Plans, a rapidly growing Managed Healthcare Company located in Coral Gables is seeking a SOCIAL WORKER.

 

ESSENTIAL DUTIES
•Participates in the evaluation of members to indentify problems, causes, and solutions.

•Establishes short, intermediate, and long term program goals based on member/family needs through home assessment interviews.

•Performs ongoing planning, organizing and coordination of interventions for the purpose of accomplishing goals based on baseline assessments.

•Evaluates program interventions and collects outcome parameters for analysis.

•Serves as a resource / case manager for the coordination of members’ psychosocial care and services; assessing utilization of resources, benefits, member/family expectations and self management skills and balancing cost of services.

Required Qualifications

 

Current Florida Social Worker license.  Proficient in the assessments of frail and elderly members regarding mental health problems, social, and community needs. Able to work independently, able to communicate effectively in oral and written forms.  Able to work within set deadlines.  Knowledge of computer software, especially Excel and Word.  Able to travel.  Bilingual is a must.
