As a Social Worker, you will:

•Responsible for making contact with patient, family, and caregiver (pfc) within 48 hours of admission.

•Provides skilled assessment of pfc psychosocial response to terminal illness and evaluates coping skills in crisis situations within five working days of admission.

•Psychosocial must become part of patient’s chart within 24 hours of completion.

•Assists pfc and agency staff in utilizing community resources.

•Assists in educating the pfc on grief and loss issues related to the terminal illness, coping skills, and community resources available to meet their needs.

•Maintains accurate on-going documentation of support referrals and the pfc responses to support referrals.

•Participates in weekly interdisciplinary team meetings.

•As a member of the health team, assists team members in understanding the significant social and emotional factors related to care of the terminally ill patient, family, and caregiver.

•Works with other team members to develop and implement the plan of care.

•Assists pfc with making funeral arrangements if requested.

•Assists in volunteer training and other educational in-services as requested.

•Provides counseling to pfc during the process of the terminal illness and at the time of death if requested.

•Meets with MSW consultant on a monthly basis for a minimum of four hours as scheduled by the Social Services Coordinator.

•Responsible for on-call rotation on a monthly basis.

•Assists in organizing and leading Bereavement Support groups and with Bereavement visits as needed.

•Expected to attend and actively participate in the annual Children’s Bereavement Camp.

Qualifications

•Master’s Degree from a school of social work accredited by the Council on Social Work Education

•Six months social work experience in a healthcare setting. If working in a Medicaid/Medicare certified home health agency, the Medical Social Worker must have at least one year experience in a healthcare setting

•Current licensure/certification in the state(s) of practice if required

•Good organizational and communications skills

•Ability to function with minimal supervision
