In all we do, we strive to meet our patients' needs and expectations.  ABOUT THE OPPORTUNITY Huguley Nursing and Rehab Center is seeking fill the Administrative Team Position of Social Services Director, Mon-Fri 8am-5pm at their 178 bed long term care facility located in Burleson,Texas. You'll be surrounded by people who are passionate about what they do.QualificationsMust have Bachelor or Master DegreeMust be licensed in the State of Texas Implement social service policies and procedures to meet residents' needs in compliance with federal, state and local regulations. Assess and  complete the sections of the Minimum Data Set (MDS) assigned completely, timely and accurately. Document the social service component of the Comprehensive Care Plan for each resident in a timely manner. Assess, plan and document residents' discharge needs in accordance with facility discharge planning policy and regulatory requirements. Encourages open communication by listening attentively and actively. Seeks to fully understand information from many points of view. Is a compelling and articulate speaker in a variety of settings. Easily adjusts the message to match the audience. Has organizational and computer skills. Keeps files and work area organized and maintains a comprehensive library of reference and historical data. Can be trusted to handle extremely confidential and sensitive information. Demonstrates persistence, overcomes obstacles, and strives to improve skills and achieve them. Ability to effectively handle escalations and time critical issues. Must be able to work closely and effectively with interdisciplinary team.
