Provides social work services to optimize patients and families through assessing, planning, educating, counseling, documenting, initiating and coordination of services and referrals in collaboration with internal and external health care team members and agencies. Provides group education and student supervision as necessary. MSW, 1 yr. health care social work practice, NJ State LCSW/ACSW. Good interpersonal skills, familiarity with medical terminology.
