Social Worker - Medical House Call Program

At Montefiore Medical Center, our Medical House Call Program provides quality primary care for  homebound Seniors with state-of-the-art diagnosis and treatment as well as basic care such a physicals, preventive screening and medical tests right in their own homes. Our House Call team is comprised of highly-trained Physicians, Nurse Practitioners, Social Workers and key support staff.

 

We are currently seeking a NYS-licensed Bilingual Social Worker (LMSW or LCSW) who has experience working with an elderly population either in a home care setting or in a Nursing Facility and a background evaluating clients for enrollment in financial support programs such as Medicaid, Meal on Wheels and other support programs. A working knowledge of Bronx-based community support agencies as well as experience working with homecare agencies securing home based services is also essential.
