This is a Full time position. 40 hours a week.

 

Key Responsibilities:
*Provide social work based case management services to Fallon Clinic physicians' high risk medical patients. Promote their health and well being while appropriately managing cost and resources.

*Maintain Case information in the Electronic Medical Record system (EPIC).

*Provide active Case Coordination and communication with the patient's care team.
*Assess patients' needs via telephone and face-to-face screening.
*Identify and coordinate with community services to address patients' unmet needs, participate in work groups and committees as directed.
*Participate with interdisciplinary team of nurses and social workers to promote the goals of Care Coordination department in serving at-risk patients.


Graduate of an accredited school of social work and licensed as a social worker in the Commonwealth of Massachusetts. Degree in Social Work required.

 

At least two years practice in a medical setting preferred. Case management experience helpful.

Computer skills (Microsoft Office) required.

Must have strong written and oral communication skills.

Should have proactive problem-solving skills, be able to adapt to changes easily and interact well with the team and clinic colleagues in a fast paced work environment.
