Supervisor, Case Management / 329 Position Description:Summary: Responsible for the supervision and oversight of Specialized Case Managers managing members with high acquityResponsibilities:• Daily supervision of the Registered Nurse Case Managers • Resource for the following activities of Case Management: assessment of need, planning of care/development of treatment plan, implementation of plan of care, monitoring and coordinating care based on treatment plan, evaluation of case management interventions, communication with member and providers, education and management of care along the continuum• Orientation of new employees to the telephonic Case Management role at APS to include: Policies, procedures, processes, workflows and desk level aides that may affect their individual team• Provide on-going performance evaluation, audits, quality measures to assist with the development of staff, individual Case Managers meeting established performance objectives, and maintaining overall departmental standards of performance• Interacts with the Director of Specialized Case Management, Medical Director and case managers as needed to solve problems and assist with complex cases• Assists in the development and revision of guidelines and protocols• Interviews, hires, train, disciplines, evaluates, counsels and terminates in conformance with personnel policies and the organization objectives• Attends required meetings such as staff meetings, case conferences • Assumes responsibility for supervision in the absence of fellow Lead case manager• Other duties as assignedEducationalRequirements:• Registered Nurse with active Georgia license; • CCM preferred or • Ability to test for CCM in 1 year (12 months from date of hire) • BSN MinimumQualifications:• Minimum of 3-5 years experience in related public sector medical services including Case Management, Disease Management and/or Quality and Utilization Review.• Minimum of 3-5 years experience supervising clinical staff;• Five(5) years of current clinical nursing in an acute care setting• Experience with chronic illness and disease states targeted by DCH strongly preferred;• Excellent verbal and written communication skills• Ability to build effective working relationships with staff and other departments• Demonstrated ability to perform critical thinking and independent decision making• Excellent planning and time management skills• Presentation skills to perform in-services• Able to handle frequent change, deal with interpersonal conflict with a mature attitude • Demonstrated ability to be assertive• Able to define and prioritize realistic goals and objectives• Excellent computer software skills• Customer service focus; and• Ability to utilize critical thinking and problem solve in a non-threatening manner
