Support Coordinator / Dedicated Case Mgr

Job Duties include serving a caseload of individuals with developmental disabilities. The Support Coordinator (SC) is primarily responsible for making contacts with those served monthly to insure their health, safety, and satisfaction with services. The SC leads the treatment team and write the Annual Service Plan. The SC must advocate for the person served and often networks to develop community/natural resources to assist the individual.

Bachelor degree is a minimum requirement in a health, social services, nursing, educ, or related field. Experience in Developmental Disab. required.

Flexibility to work from your own home and set your own schedule. Bonus package offered with mileage reimb.
