As a Therapy Team Leader, you will:
•Direct care/services provided and development/implementation of patient plans of care and physician protocols for Specialty patients within scope of practice, to include performing admissions screening, clinical assessments and treatments as appropriate, consulting with third party representatives regarding patient financial responsibility, providing case management and follow up, to ensure care plan stated outcomes are achieved.
•Proactively facilitate coordination of ancillary/paraprofessional services and assists with coordination of disciplines and utilization of community resources, including transition of care from a facility to place of residence as needed. Oversee and assist in evaluating the performance of all team members which may include therapists, social workers, nurses and HHA’s. Collaborate with team members to ensure an appropriate match between patient needs and the skills/abilities of the care team.
•Coordinate care/communication between team members, attending physicians, appropriate administrative staff, referral sources and case managers to ensure appropriateness of care, and facilitates any necessary changes to the plan of care. Review and interpret trend over time data/patient progress reports and provides concise findings to physicians, ARNP’s and PA’s.
•Promote specialty growth in revenue and profitability. Manage resource allocation (care/services and supplies) within budgetary and Care Protocol restraints or documents appropriate variances. Maintain marketplace Specialty information to assist the Specialty/Rehab Director in assessing business opportunities. Provide input to quarterly/annual business/market plans and budgets, cost analyses, monitors expenses and explains variances to budget.
•Assist Sales team with Clinical review of Specialty for customers or any other related marketing duties, including meeting with potential customers to develop relationships.
•Collaborate with RN Clinical Manager to manage the relationships between service demands and availability of Specialty credentialed staff.
•Participate in the recruitment/interviewing/selection/orientation of team members, evaluates their performance relative to job goals/requirements, coaches staff recommending in-service education programs and ensures adherence to internal policies/standards. Promote ongoing Specialty development through on-site observation of care delivery. Provide Specialty training/credentialing of all Specialty team members.
•Ensure that the scheduled weekend and after hours visits including holidays are covered for the Specialty team's case load to ensure optimum care is provided without interruption. Participate in on-call schedule as needed.
•Ensure compliance with standards/company policies/procedures and external regulatory requirements. Ensure that caregiver clinical documentation meets these requirements.
•Maintain current/ongoing clinical knowledge through internal/external training programs, provides interpretation of knowledge and direction to staff. Participate in “train the trainer” opportunities related to Specialty practices and new clinical developments as an opportunity for self-development and to support growth of Specialties as required.
Qualifications
•Degree and licensure in the state (s) of practice for either physical therapist, occupational therapist or speech language pathologist with a minimum of three years clinical experience required to include; home health, hospital or skilled nursing facility experience in at least one of the following: Rehabilitation, Medical Surgical or Sub-specialty Units such as Oncology, Endocrine, Orthopedic, Cardiopulmonary, Gerontology or Neurology preferred.
•Experience leading an interdisciplinary care/rehabilitation team and demonstrated awareness of individual discipline’s roles and abilities required.
•Experience in a community/homecare setting and in the Specialty of the Team Leader role preferred. One year in a supervisory role generally preferred.
•Current CPR certification also required.
