Treatment Adherence Specialist/ Medical Case Manager

REPORTS TO: Clinical Program Director

BENEFITS:

To implement a treatment education and adherence program that has been selected as a model program for other community-based organizations across the country.  Help patients navigate the health care system, facilitate access to medications, ensure clients understand laboratory and other clinical studies/tests. Ensure clients are on their own treatment team and understand their treatment goals and treatment plans.

EXPECTED JOB ATTENDANCE:

38 hours per week (M -10:00 a.m. – 7:00 p.m., T, Wed. 9:00 a.m. – 6:00 p.m., and Thurs. & Fri.  9:00 a.m. – 5:00 p.m.)

 

Salary Range:

$60,000 to $75,000 (health, dental, life insurance, and 403(b))

 

SPECIFIC DUTIES:

 

· Work with HIV Specialist(s) on management of patient medical issues.

· Provide after hours support for CVC clients in concert with HIV Specialist.

· Provide backup triage services when HIV Specialist is absent.

· Provide telephone contact to patients to discuss labs and other medical test results, as well as, returning patient calls.

· Assist with specialty referrals and tracking of specialty visits.

· Assist patients to access pharmaceuticals via programs such as patient assistance programs.

· Assist with implementation and monitoring of clinical quality improvement efforts.

· Participate in clinical team meetings and multidisciplinary meetings.

· Assist HIV Specialist and Medical Assistant with ordering, maintaining, and inventory of supplies, medicines and vaccines.

· Plan, coordinate, implement and evaluate a comprehensive treatment education and adherence program that includes one-on-one counseling, a peer support group program, and a volunteer buddy network;

· Develop written educational materials as needed to inform clients about treatment regimens, particularly the need for compliance with medications prescribed;

· Manage weekly/monthly schedules for treatment counseling/interventions;

· Responsible for data collection to determine effectiveness of treatment support services;

· Provide written consults to client’s primary care providers on client status as it relates to Adherence Counseling and Treatment Education.

· Coordinate and co-facilitate workshops, seminars and support groups with the Mental Health Specialist;

· Attend appropriate training to stay current with HIV drug treatments, therapies and developments; and

· Perform other related duties as assigned by the Clinical Program Director.

QUALIFICATIONS:

 

· A Registered Nurse, Nurse Practitioner and/or Physician Assistant with a minimum of one year of specialized experience working in HIV related health education programs;

· A strong knowledge of classes, usage and contraindications of HIV drugs is required;

· Demonstrated ability to work with diverse populations and groups; and

· Experience in addressing specific HIV educational and behavioral-based interventions.
