The clinical pharmacist recommends and monitors patient drug therapy and provides drug information. The clinical pharmacist interacts with patients, family, and other health care providers as appropriate to assure continuity of services and to achieve desired patient outcomes through safe, timely, cost-effective drug therapy. Performs the duties of staff pharmacist as required. Skills/Requirements KNOWLEDGE, SKILLS AND ABILITIES:Has the ability to communicate clearly, and effectively, possesses excellent oral and written communication skills and can relate to diverse populations in an open, friendly and accepting manner; is able to make timely and sound decisions under conditions of uncertainty; can handle stressful situations; able and willing to travel occasionally as well as attend evening and weekend events. Able to manage conflicting priorities; strategic thinking; problem solving, team building; comfort level with ambiguity and autonomy; able to facilitate effective working relationships to achieve established goals and objectives; role models values of St David’s Healthcare Partnership. Must be able to demonstrate understanding of HCA’s and St David’s patient safety initiatives and adhere to strict compliance to all safety protocols and procedures.EDUCATION AND EXPERIENCE:Required: BS in Pharmacy or PharmD degree. At least 5 years clinical experience in all aspects of comprehensive hospital pharmacy practice.Preferred: Advanced education desirable (i.e., Pharm. D., Masters Degree, ASHP accredited residency). Knowledgeable in Pharmacy Computer entry and Medication Dispensing Systems. Board Certification.LICENSES AND CERTIFICATES:Required: Pharmacist licensure in Texas.
