May assist patients with using monitoring devices for the first time. ESSENTIAL FUNCTIONS: The tasks, duties and responsibilities of the position that are most important to get the job done. Verbal Consent Duties: · Contact patients daily by telephone to obtain enrollment in Our Disease Management Programs. · Conduct telephone interviews with referred patients to determine their appropriateness for enrollment and participation in our Disease Management Programs. Confirm patient eligibility using specific criteria and following documented procedures, policies and guidelines. · Educate potential enrollees -- answer questions from potential enrollees and obtain verbal consent from patient and ensure the patient has been placed in the highest service level qualified. · Enter patient information into our proprietary database; keep records updated at all times. · Assure that all members/clients/patients/ are treated with respect and consideration, that their right to treatment or service is respected, that confidentiality, privacy and security are maintained, and that they are involved in all aspects of their care.Establish Connectivity Duties: An additional assignment based on business needs and competencies. · Contact patients that have yet to submit first monitoring measurements and provide assistance as needed to ensure measurements are attained by us. · Provide fundamental troubleshooting assistance as necessary. Engage the patient in understanding the problem and identifying appropriate solutions. · Able to overcome objections and re-engage the patient in finding value in the program to ensure continued commitment and service. · Properly use resources to ensure accuracy in the set-up process. Follow documented policies, procedures and guidelines. Other Duties: · Promote effective coordination and communication across disciplines and departments within the organization. · Participate in continuing education activities to strengthen and broaden skills and knowledge base. · Meet or exceed all departmental standards and expectations. · Perform other duties as assigned. WORKING CONDITIONS: The environment in which the job is performed, especially any unique conditions outside a normal office environment, i.e. extensive travel, high noise levels, frequent lifting of over X pounds. The Enrollment Services Representative works in a normal office environment. Moderate physical exertion. Uses hands to manipulate equipment, supplies, computer and telephone. Standing, walking, gripping, fingering, bending, lifting, reaching at least 50% of work time. Occasional lifting of up to 20 pounds. Extensive computer data entry. MINIMUM QUALIFICATIONS: The specific knowledge and skill requirements and the minimum level of work experience and/or education required to perform the job. Education: High school diploma or equivalent. Experience: Minimum two years inside sales experience (health care preferred).
