ABOUT THIS OPPORTUNITY
In this role, you will get to lead and mentor a dedicated team of nurse care coordinators to ensure care coordination and disease management activities and requirements are fulfilled for the customer and beneficiaries. It also provides you the opportunity to improve policies and procedures to drive efficiency and quality.# of People on Team: 17 # of People in Dept.: 60Typical/Expected hours of work: 40 – 45 hours; no weekendsTypically works with: internal employees, United Mine Workers MMS leadership, clients, vendors and beneficiaries Travel: 3 – 4 nationwide trips per year to see the customers/clients
Responsibilities:
1. Supervises Care Mngt./Disease Mngt. staff, including general human resource duties and assisting with the hiring and disciplinary process. Oversees the day-to-day workflow and quality of the work being completed by the Care Mngt./Disease Mngt. nurses. Ensures work is completed in a timely manner to meet customer expectations. Elevates issues as needed.
2. Oversees day-to-day operations, including review or analysis of reports and caseloads as well as identification and follow up regarding quality issues. Identifies the need to escalate cases or issues to supervisor.
3. Identifies, develops, and makes recommendations regarding the on-going needs of the Care Mngt./Disease Mngt. area. Provides input to management towards solutions to issues. Develops new and on-going procedures and policies if necessary for quality of care for the beneficiary, client satisfaction and staff quality and consistency. This includes revising existing policies, procedures and process flows as necessary to drive efficiency.
4. Manages and leads projects to a timely completion.
5. Provides input towards system redesign for improved efficiency and quality.
6. Other duties and projects as assigned.
Qualifications:Bachelor’s degree in nursing required, BSN preferredCurrent, valid and unrestricted RN licensure in State of Minnesota requiredCCM (Certified Case Manager) and/or Geriatric Case Management certificate required or ability to obtain within 6 months of employment3 years supervisory or management experience in an operations setting, preferably health plan administration, managed care, or other clinical or health-related fieldMinimum of 2 years experience in care coordination, case management, or disease managementMinimum of 5 years work experience with older adult population in a community setting, long-term care facility, managed care, or other health related field preferredDetailed knowledge of care coordination or disease management theories and techniquesAbility to assist with and provide direction in the development of care plans based on identified care needsAbility to lead staff, including ability to organize, prioritize, motivate and promote team workExcellent written and verbal communication skillsAbility to analyze processes and procedures and make recommendations for modifications or additions to theseFlexibility to adjust to fluctuating workloads and changing assignmentsProficient in Microsoft Office (Outlook, Word, Excel)Ability to provide excellent customer service to internal and external customers/clients
