Care Mgmt Nurse Supervisor - RN #23621
Team leader is responsible for management of clinical and non clinical staff including organization and development of high performing teams.  Facilitates effective interface among team members.  Accountable for meeting the operational and quality objectives of the team.  Performs telephonic disease management assessments when needed to assist team.

Required Skills:
-Current RN license to practice in the United States
-Three to five years clinical nursing experience
-Minimum 2 years managed care experience
-Previous management experience preferred

RN Care Managers #23624
Responsible for managing patients in a telephonic disease management program.  Screens members to identify needed medical services, modifiable risk factors and educational needs and identifies or refers cases for other services.  Intervenes to positively effect healthcare outcomes.  Applies data driven methods of identification of members. Conducts individualized clinical assessments of members care needs and develops a personalized disease-specific action plan.

Required Skills:
-Registered Nurse
-3-5 years of clinical experience
-Disease Management, Case Management or Managed Care experience preferred

Case Management Nurse Consultants - RN #23346

-Certification in Case Management preferred
-Previous experience in case management or utilization management
 
LPN Consultant #23345
Responsible for answering incoming calls and initiating outbound calls for enrollment in the Informed Care Management Program, including determination of plan requirements and initializing cases. Involves a combination of customer service capabilities, personal skills in communication, computers and problem solving ability. Must be able to receive incoming calls and make outbound calls with a pleasant telephone manner and assist the caller in an extremely fast-paced environment.  Applies appropriate clinical criteria and guidelines while conducting individualized assessments of member care needs.
 
Required Skills:
-LPN with 3-5 years of clinical experience
 

We are seeking staff for all shifts M-F 7:00 a.m.-10:00 p.m. and rotating Saturdays 10:00 a.m. – 3:00 p.m.   We are very interested in multilingual candidates, especially those with the ability to read and write Spanish fluently.
