Alcanza Case Manager

The Case Manager works under the direct supervision of the Social Work Coordinator. In the supervisor’s absence, the Program Manager supervises the position. The position of Case Manager requires a sincere interest in working with and supporting the overall health and development of teen mothers, and their children and families. The Case Manager is responsible for working as a team with their supervisor to serve clients as follows: providing case management services, ongoing assessment, the development and/or review of service plans, responsibility for a caseload, documentation, referrals and follow-up, advocacy and submission of programmatic and statistical reports. Requirements: Bachelors degree in Social Work, Psychology, Human Services, Education or related field; Two –five yrs. of experience in case management, human services; Bilingual (Spanish/English) preferred; Knowledge of prenatal care, postpartum care; Understanding of teen pregnancy and adolescent development; Strong crisis intervention
