CASE MANAGER

Concern for Independent Living is a non-profit provider of housing and services for disabled individuals and families. Serving more than 450 individuals and persons in families throughout Suffolk County, we are one of the largest housing agencies of this kind on Long Island. Concern offers a variety of housing and comprehensive services, delivered in a manner that is sensitive and allows our clients recovering from mental illness to live with dignity in the community. 

CASE MANAGER needed for a residential housing agency in Medford, Long Island.  The Case Manager is responsible for providing case management services directly to clients.  The Case Manager is considered a resource in that they should be familiar with community resources and services to which they may help their clients gain access.   Bachelors degree in Psychology, Social Work or other related mental health discipline plus two years of experience working in a similar setting and/or working with people with mental illnesses.Great benefits including paid time off, life insurance, medical, dental, tuition and mileage reimbursement and pension!  Send resume to sleiva@concernhousing.org or fax to (631) 758-0467.
