CLINICAL COORDINATOR - Supportive Housing Program - Position in Middlesex & Ocean Counties

Job Summary:

Under direction of the Director of Outreach Services, coordinates outreach  activities related to program development & consultation; manages hospital & community referrals and intake; supervises support staff; provides on call coverage. Must be willing to travel between Middlesex and Ocean Counties.

 

Responsibilities:

• Provides supervision to assigned Outreach facilities via direct supervision and oversight.

 

• Oversees the delivery of quality mental health rehabilitative services; including but not limited to, activities of daily living (ADL), case management, linkages, advocacy, recreation, socialization,  vocational and educational supports, symptom management, medication education and management, and obtainment of medical/care/services.

 

• Ensures the efficient and accurate completion of documentation in accordance

with agency, state and federal guidelines.

 

• Provides on-call coverage as per agency policy and is available to respond to emergencies, both clinical and administrative as necessary.  

 

• Participates in the design and implementation of outreach/supportive housing programming to meet the individual and group needs of agency consumers.

 

• Receives referrals for services and admission.  Keeps the Waiting List for placements.  Screens candidates for admission to programs.  Refers inappropriate referrals to appropriate community service providers.

 

• Provides individual, family, group and rehabilitative services as appropriate to meet consumer needs; makes appropriate referrals as needed during course of treatment.

 

• Performs other duties as assigned by supervisor.

 

Requirements:

Master’s degree plus one year of experience in mental health setting; knowledge of psychiatric rehab. and area resources a plus.  Valid NJ driver’s license.
