Children's Case Manager

RESPONSIBILITIES:

 

§ Assess and develop plan to address child’s developmental and therapeutics needs.

§ Provide individual counseling services to children such as play therapy, family therapy or other therapeutic interventions as determined by the needs of the client.

§ Consult with and make recommendations to the mother, school personnel, the mother's Case Manager, regarding the needs of the child.

§ Develop and facilitate therapeutic groups for specific age children.

§ Establish supportive relationship with children's mothers in order to provide parent intervention when necessary.  Specifically in areas of discipline, nurturing, communication and socialization.

§ Develop and implement procedure for referrals to community resources.

§ Conduct intakes and assessments for children.

§ Prepare and present cases in staffing.

§ Work as a member of a team to insure appropriate case management for assigned case load.

§ Provide adult supervision for activities.

§ Maintain accurate records on each child for assigned case load, (progress notes, reports, and other required forms).

§ Respond appropriately to subpoenas.

§ Assist in facilitating parenting groups for women.

§ Provide crisis intervention services as needed.

§ Work with program volunteers

§ Provide safe transportation of children when necessary. This includes driving agency vehicles, or in the event of an emergency, driving personal vehicles.

§ Represent The Family Place and participate in community education as requested.

§ Conduct home visits as required.

§ Participate in training programs and workshops to update professional knowledge.

§ Meet regularly with supervisor as arranged. 

§ Provide reports and/or accurate statistical information in a timely manner.

§ Maintain work schedule as required by program needs with adequate flexibility

§ Perform all other duties as assigned by supervisor.

 

REQUIREMENTS:  

 Masters Degree in Child Development, Social Work, Childhood Education, Counseling or related field and licensure required (e.g. LMSW, LCSW, LPC, LPC-I, etc.). One year experience in a therapeutic setting with children preferred.  BILINGUAL SPANISH/ENGLISH PREFERRED.  Good driving record and valid driver’s license required.
