Description of Duties:
We are seeking a person to provide direct mental health services to individuals participating in the Thresholds 6710 North Program. This program serves young adults, ages 18-30, who reside in a clustered apartment setting on Chicago’s northeast side. Here, the Community Support Specialist works as part of a team to help program participants develop independent living skills and establish and pursue personal life goals in the context of a supportive residential program community. Responsibilities include: provision of group and individual services, developing care plans and overseeing their implementation, linkage to internal and external resources, advocating for participants in the community, working with families as part of the treatment team, and maintaining clinical records and documentation.

Qualifications and Experience Required:
A successful candidate will have experience working with persons with mental illnesses. Qualities include the ability to facilitate groups and social activities; clear and effective verbal and written communication skills; the ability to be flexible and culturally sensitive; and the motivation for continued learning and professional development in a dynamic field.
 
Requirements, Responsibilities and/or Duties Unique to the Position:
Must have a valid Illinois driver license and have daily access to a vehicle. Basic MS Word and general computer skills are an essential part of this position. Driver's license, automobile and 100/300 auto liability insurance
