Home Care Case Manager/Bilingual Spanish

$500 Hiring Bonus

Empower elders to remain independent in the community, serving as a client advocate by providing support and coordinating services as needed.  Perform in-home assessments; determine eligibility, develop, manage and monitor service plans.  Bachelor’s degree in Human Services or related field with 1-2 years experience.  Must be LSW eligible and license obtained within 6 months.  Mon-Fri 35 hr work week.
