Intensive Job Coach - Supported Employment, Adult Rehabilitation Services
The Brooklyn Bureau of Community Service is one of Brooklyn’s largest and oldest non-sectarian social services agencies. Since 1866, the Brooklyn Bureau has been a leading provider of social services to families and children, and to adults with physical and developmental disabilities, as well as those with a history of mental illness. Serving more than 13,000 individuals in 2007, the Brooklyn Bureau provides a broad and diverse range of services including prevention of foster care placement; crisis intervention; early childhood and after school education; vocational training, job placement, and clinical services to adults. The Supported Employment Intensive Job Coaches assist disabled consumers with finding and retaining employment through active, individualized job search and preparation, and post-employment job coaching at the work site. Intensive Job Coaches help consumers to achieve job stabilization and transition to Supported Employment Extended Services.Responsibilities: •Help consumers with mental retardation/developmental disabilities (MR/DD), learning disabilities, mental illness, and/or physical disabilities to achieve employment goals by assisting them with job matches that meet their interests and strengths.•Identify with participants their potential gaps in performance and help them to develop individual disclosure plans. With participant’s permission, assist with disclosure to employers and co-workers. Advocate for needed accommodations through job modification. Help participants develop natural supports from family members and co-workers.•Provide on-site worker instruction and identify individual learning needs or cognitive deficits. Develop strategies for effective learning and supports for task completion; instruct the job site supervisors of the needs and strengths of the participants. Provide travel training, case management, and services to support job retention.•Arrange for clinical services for participants with mental health conditions.•Assist participants with obtaining Medicaid for healthcare and services, as needed, and begin enrollment in Medicaid Service Coordination program for individuals with MR/DD, to maintain waiver service eligibility.•Work with current employers to expand employment opportunities. •Complete necessary case record documentation and VESID monthly reports. Requirements: Minimum High School Diploma with related experience. BA preferred. Good verbal and written communication skills. Excellent judgment and sensitivity to the needs of the consumers.
