Juvenile Justice Specialist

GENERAL DUTIES:

The Juvenile Justice Specialist is responsible for facilitating the provision of family preservation and support services to families eligible for service through Wayne County Department of Children & Family Services in collaboration with Bridgeway Services. 

 

SPECIFIC DUTIES:

 

1.     Coordinate the care for youth placed with the Wayne County Department of Children & Family Services.

 

2.     Orientates youth and families to the policies and process related to their care as identified by Department of Children & Family Services and Southwest Counseling Solutions.

 

3.     Works a 37.5 hour non-structured work week that may include evenings and weekends, depending on the needs of the assigned family caseload.

 

4.     Assists family in identifying Child and Family Team members, which includes family members, providers, and professionals and identified support, to assist in treatment planning process, BARJ goals and support.

 

5.     Facilitates the completion of strength, needs assessments and creating an integrated Plan of Care.

 

6.     With the Child and Family Team, creates a 24-hour crisis plan.

 

7.     Maintains at least weekly face to face contact youth placed in the community and at least monthly face to face contact with youth placed in out of home care. 

 

8.     Maintains at least monthly contact with family.

 

9.     Facilitates the family’s access to services set forth in the plan, including traditional and non-traditional service.

 

10.   Monitors the implementation of the Treatment Plan of Care.

 

11.  Meets at least monthly with the Child and Family Team.

 

12.  Tracks and monitors days of care and treatment.  Ensure accurate billing as well as identifying and submitting care management track information.

 

13.  Twenty-four hour emergency availability on rotating basis.

 

14.  Recommends closure of a case due to achievement of the family's goals and recommends referrals to community-based services.

 

15.   Provides documentation for all services delivered in accordance with agency

       standards and guidelines established by funding sources.

 

16.   Files all documentation within 24 hours of receipt or rendering services.

 

17.   Consults with Supervisor at least biweekly on all assigned family cases.

 

18.   Serves on agency committees, work groups and other bodies as assigned.

 

19.   Maintains monthly contact with services providers. Monitors services and

       treatment plan development of providers.

 

20.  Maintain client case record in clear, organized manner by obtaining, reviewing and filing (within 24 hrs of receipt) necessary documentation in a timely fashion.

 

21. Participate in regularly schedule clinical consultation.

 

22. Update and maintain client electronic record on Juvenile Assessment and

      Information system (JAIS).

 

23.  Familiarity and adherence to Wayne County Department of Children & Family Services, Juvenile Justice Services handbook.

 

24.  Familiarity and adherence to Bridgeway Service policy and procedures and   Professional Plan of Service.

 

25.  Monitors and facilitates educational related goals including admission, attendance and educational testing.

 

26.  Adheres to SWCS Code of Ethics.

           

27.  Adheres to SWCS policies and procedures.

 

28.  Complies with the agency standards for Service Delivery.

 

29. Performs other duties as assigned by the Supervisor.

 

KNOWLEDGE, SKILLS AND ABILITIES:

 

1.               Ability to function effectively as part of a professional team.

 

2.               Knowledge of and ability to adhere to a professional code of ethics.

 

3.               Knowledge of and ability to comply with Michigan Mental Health Code and HIPPA.

 

4.               Flexibility in days and hours available for scheduled work, including evenings and weekends.

 

5.               Knowledge of community resources and supports for families with special needs.

 

6.               Ability to function independently in client's home and in the community.

 

7.               Knowledge of child and adolescent development and ability to work with families from diverse communities.

 

8.               Ability to type and knowledge of basic computer applications (MS Word and MS Outlook)

 

MINIMUM QUALIFICATIONS:

 

1.               Master's degree in mental health/child welfare related field preferred or a Bachelor's degree in mental health/child welfare related to field with a minimum of four years related experience.

 

2.               Possession of a valid driver’s license and access to a private vehicle for day-to-day job performance.

 

3.               Fluency in Spanish preferred.
